For the purposes of this document, the terms Patient, Client, Student, Customer, Enrollee, Registrant, and/or any other subscriber to our services may be referred to as Participant in this and/or other documents.

Terms Of Registration For Courses, Classes Etc. :
It is our goal to make everyone’s visits (virtual or in person) a wonderful experience. We are committed to helping you achieve your goals and provide an atmosphere conducive to our integrative philosophy of addressing the whole individual (mind, body, and spirit). To aid in our commitment it is important that participants cooperate with the following office / Online / event policies. Thank You!
Payment
Whether paying with cash, check or credit card it is required that we have a valid credit card on file. If the credit card that you are using is that of someone else, their information (address, phone number, email, etc.) must also be included on our credit card form. The participant in signing this form acknowledges that any unpaid balance or other money owed may be charged to this card if their account is not current. This includes charges for missed appointments. Payment for services are due either at the time services are provided or prior to that time depending on the program you have selected, unless other arrangements are approved in advance by our office management in writing. For therapy, counseling, life coaching, mentoring etc. a significant amount of time is set aside for your appointment. Credit cards for all new patient’s will be charged prior to the initial visit so that payment clears prior to the date of that appointment.
Excluding calls made for the sole purpose of scheduling an appointment(s) telephone calls/consultations lasting more than 5 minutes are billed as an appointment. Please make an appointment to discuss your case etc. (Your provider’s time and knowledge are their stock and trade).
Note: There is a $50.00 charge for checks returned un-payable for any reason or for credit card charges denied. This document authorizes Masters Center to bill your credit card for any outstanding payments due.
Late, Missed and/or Changing Your Appointment
We attempt to see our participants on time. If someone is late for his or her appointment, the next person is then forced to wait past his or her scheduled time, therefore, 15 minutes late or more may be considered a missed appointment. If this occurs, you may forfeit your appointment time with no refund, at the discretion of the provider. For any appointment cancelled by the participant before 8:00 AM on the day of the scheduled appointment there will be charged $50.00 late cancellation fee. For any cancellations after 8:00 AM on the day of the scheduled appointment, the full fee for the visit will be charged. Your time is set aside specifically for you. As well, it is normally impossible for us to fill a cancellation on short notice so… please be on time for your appointment or you may be charged for the missed appointment (or lose a session if it has already been paid for. We require 24 hours’ notice to change or cancel an appointment.
Online appointments require that the participant be logged in and have all necessary equipment functioning 15 minutes PRIOR to appointment time.
Refund Policy
All sales (programs, appointments, services, sessions, workshop, merchandise) are final. We will gladly replace any defective product with one that is either identical or as close to identical as possible if it is brought to our attention within 48 hours of delivery (Physical Item or Digital Item).
OTHER POLICIES
Re: In person appointments – No unattended children under the age of 16 years old can be left unattended in the reception area or the outside halls. This is for liability reasons and as a courtesy to other patients, students and guests. Participants may not bring another person to share in or piggy-back with their appointment. Anyone wanting services must schedule their own separate appointment.
Re: In person appointments – Out of courtesy to other attendees/participants and to maintain the serene atmosphere that we strive for, there is absolutely no cell phone use permitted in the office. If you must make or take a call, or want to surf the internet or play games, please step into the hallway.
Hold Harmless
Registrant/Participant accepts full responsibility for payment of their fees. Client/ Participant holds harmless, indemnifies and agrees for his/herself and his/her heirs and assignees not to sue Masters Center LLC and/or any owners, agents, staff or associates arising out of his/her participation and/or presents either in person or online, or other and affirms that he/she is participating of his/her own free will. Participant(s) are responsible for any reasonable legal fees incurred by the provider in the collection of money owed or any damage caused to the provider financial or otherwise. If any part of this agreement is deemed unenforceable all other parts of this agreement remain in full effect.
IMPORTANT: It is prohibited for ANYONE to bring and/or use recording devices whether in person or online, at any class, session, meeting, event or other connection. Masters Center LLC reserves the right to discontinue services to anyone found doing so with no refund of fees paid. Any balance remaining will still be owed. Masters Center reserves the right to confiscate, inspect and delete any information recorded without permission, after which, the device will be returned. Masters Center LLC is not responsible for any damage to the device(s) if this should occur.
Phones must be completely turned off during any class, session, meeting, or event. Interruptions caused by phones may result in the immediate ejection from the event with no refund of fees paid. Any balance remaining will still be owed.
Dr. Holder and/or Masters Center LLC reserves the right to record (audio/video) any sessions, events or consultations for our records. Any recordings become the sole property of Dr. Holder and/or Masters Center LLC. Recordings may be used by the provider to review a case or for use in educational training media. With such recordings as well as all other patient client records, the client or patient’s identity will be protected and kept confidential.
Registration Session and Intake Fees / Session Fees And Programs
Dr. Holder’s Fees: (Including: Hypnotherapy – Counseling – Life Coaching – Mentoring)
Note: Enrolling in a program is most often to the participant’s advantage. Programs are discounted below the standard base session prices corresponding to the length of the program.
New or returning participant’s must complete an intake form and have had their Intake Session before any sessions and/or programs can commence.

We begin assembling your chart and file immediately after receiving your registration form and/or scheduling your appointment, therefore, a $200.00 non-refundable registration fee will be charged to your card when we receive your completed registration form, or on the day that you schedule a date and time with us for your appointment (whichever comes first). This applies to all new participants and to all returning participants (who have not seen their provider in over 12 months). Your initial appointment (The Intake) can last up to 2 hours. The fee for your Intake is $250.00 which will be billed to your credit card prior to your scheduled appointment

3-Session program paid in full at the time of enrollment – only $865.00
6-session program paid in full at the time of enrollment – only $1050.00
9 -Session program paid in full at the time of enrollment – only $1325.00
12 -Session program paid in full at the time of enrollment – only $1655.00 (Great Value)
14 – Session program paid in full at the time of enrollment – only $1925.00 (Best Value)

Session Base Fees For Single Sessions (Not part of a program)
Session base prices with Dr. Holder:
$475.00 per regular session.
$275.00 Reinforcement Session.

Urgent Care or Off-Hours Fees
Urgent Care or Off-Hours Single Session Pre-Paid Price – $575.00 for maximum of up to 3 hours.
Anything over 3 hours is billed at an additional $3.95 per minute.
Our standard intake/registration forms are required to be filled out by the registrant, prior to the appointment.
Sessions that are part of a program cannot be used for Urgent Care. Urgent care fees must be purchased separately.

Regression Therapy and PL Regression (If not included as part of a pre-paid program):
Option 1: Standard Practice is a 2-session program, each session lasting up to 2 hours each. Fee: $750.00
Option 2: Fee of $450.00 per session, if paying separately for each session.
Re: All regression sessions that are not conducted within the context of a regular non-regression program: Any additional time after the initial time allotted for the session, will be billed by the minute at $5.50 per minute.
Memory Retrieval (Lost items etc.): 2 sessions program. (Session 1: 1- hour Intake/Prep. – Session 2: Up to 1 ½ hours for Memory Retrieval session). $395.00.
NOTE: There are no guarantees with any type of regression session or memory retrieval for lost items, or any other issue involving regression or memory retrieval and/or enhancement. There are variables over which the provider has no control. (Including but not limited to, subject’s fear of finding a particular outcome. Items being stolen, not lost. Items falling from a hole in a pocket with no awareness of the event by the subject, etc.).

Calls And Other Communications
For Emergency Calls, emails, etc. and other contact outside of session/appointment times.
If you have encountered an emergency related to your services, your provider will always attempt to make time to speak with you by phone or online. If your provider is unable to take your call or to email, text or otherwise respond at the time you attempt to contact your provider, your provider will return your call (or other contact) as soon as possible. If you are having a medical emergency, call 911 or call your nearest hospital emergency room. Other than emergency situations, participants are asked to keep questions etc. confined to their scheduled appointment time, or you may speak to an office staff member about general matters. Any contact (phone, text, email, etc.) taking more than 5 minutes of the provider’s time and resources will be considered a consultation and will be billed accordingly at $2.50 per minute with a minimum charge of $50.00).

OTHER TERMS, POLICIES AND CONDITIONS
Missed and/or Late For Appointment
Regarding Late and/or Missed Appointments: Your provider’s time and knowledge are his or her stock and trade. Your provider is setting aside over 2 hours of his or her time for your initial Intake appointment. It would be difficult if not impossible to fill that time if cancelled at the last minute, or by failure to arrive on time for the appointment (On location and/or Online). An intake appointment can be cancelled or changed, however, changes and/or cancellations require 24 business-hours’ prior notice. If your appointment is cancelled with proper notice, a refund of the deposit, less a $50.00 clerical fee, will be refunded to the registrant’s credit card. Any cancellations made with less than the 24-hours required, will result in forfeiture of the full registration and/or session fee so… Please contact us in advance if you need to change your appointment. Subsequent scheduling changes or cancellations for regular appointments require 24 business hours prior notice. At the provider’s discretion, you may be charged a service fee of $50.00 if 24-hour notice is not provided by the participant. If a participant, cancels after 8:00 AM on the day of the scheduled appointment, or if a scheduled appointment is missed entirely, the participant, will be responsible to pay the full amount for the missed appointment (or if they have pre-paid program sessions, they will forfeit a session).
Participant Status
Participants are no longer considered “Active” if they have not been seen for services within the previous 12 months.
For appointments at our physical location only: In inclement weather, participants must call in advance to cancel. Do not assume that we will be closed due to inclement weather. Also, you may have the option in some cases to change your appointment from an on location session to an online session.
Payment Method
We accept cash, debit card, credit card, Venmo and CashApp (no checks please). Registrants must have a valid credit card on file and authorized signature (or e-signature) allowing us to bill for appointments and/or merchandise. Those required to complete an intake form for services are providing this authorization and no further action is needed. All session packages have been discounted. All services must be prepaid unless other arrangements have been authorized by your provider in writing.
All conditions stated in the initial intake form, filled out by the participant, apply to both physical and virtual sessions, and can only be waived by the provider (in writing).
All program fees are discounted, therefore. All program sales are final and are non-transferrable.
No electronic recording of any type is permitted without advance written permission from the provider. If permission is given by the provider in writing, the provider’s, “1-session Usage/Privacy/Copyright Agreement” must be signed prior to the session or other appointment and returned to the provider by the participant prior to that session. The “Usage/Privacy/Copyright Agreement” is for 1-session only and expires at the end of that session.
Any and/or all discounts and/or waived fees may be withdrawn, reversed, and deemed null and void if a participant makes any changes, modification or cancels the purchase of any service, program, class, product, or other item. The participant(s) will then be responsible for paying the recalculated cost of any cancellation fees, used portion of purchase or any other standard fee(s) or costs at the most current standard rate(s) of Masters-Center LLC.
During session(s), only the participant of record is permitted in the room that is being used for the session unless advance written permission is given by the provider. If permission is given, the provider’s guest will be required to sign a document agreeing to follow the same office policies and rules as the participant.

Any changes and/or amendments will be posted on our website. Updates and/or changes posted on our website will amend any preexisting terms and policies.
